PO-05-060 SUBCUTANEOUS VERSUS TRANSVENOUS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR THERAPY: A META-ANALYSIS
نویسندگان
چکیده
Subcutaneous implantable cardioverter defibrillators (S-ICD) are safe and effective for the prevention of sudden cardiac death. To compare safety efficacy S-ICD with transvenous (TV) ICD. Database search was performed all RCTs and/or propensity score-matched (PSM) studies comparing against TV-ICD. The primary outcome lead-related complications. Secondary outcomes included device-related complications, appropriate inappropriate shocks, all-cause mortality, deaths. We 6 (2 4 PSM) a total 2,890 patients (1,446 vs 1,444 TV-ICD). mean age 54±15 years, 23% were female, 47% ischemic cardiomyopathy, 22% had atrial fibrillation. EF 37±15% 75% ICD prevention. Compared TV-ICD, associated significant reduction in complications (OR=0.13; 95% CI=0.05 – 0.36; p<0.01; NNT=23) (OR=0.56; CI=0.32-0.99; p=0.05; NNT=34). There no difference shocks (OR=1.16; CI=0.70-1.70; p=0.44). Individual trials’ OR shock ranged 0.48 2.38 (no individual trial statistically this endpoint), likely due to heterogeneity device tachytherapy programming. differences between groups regards or deaths (p>0.05). Among ICD, significantly lower rates lead- compared mortality.

متن کاملSubcutaneous implantable cardioverter-defibrillator placement in a patient with a preexisting transvenous implantable cardioverter-defibrillator
Introduction The subcutaneous implantable cardioverter-defibrillator (S-ICD) is a safe and effective treatment for prevention of sudden cardiac death. It is typically not used in patients who require additional therapies conferred by transvenous implantable cardioverter-defibrillator (TV-ICD) systems, such as bradycardia pacing or antitachycardia pacing (ATP) for ventricular tachycardia (VT). D...

or nearly 3 decades, the implantable cardioverter-defibrillator (ICD) has been available to patients who survived life-threatening rapid heart rhythms or are at risk of experiencing them. The ICD comprises a device generator coupled with a defibrillation lead. Traditional ICDs are implanted under the skin with the generator positioned beneath the collar bone. The defibrilla-tion lead is inserte...
متن کاملTransvenous Implantable Cardioverter‐Defibrillator (ICD) Lead Performance: A Meta‐Analysis of Observational Studies
BACKGROUND Despite the widespread use of implantable cardioverter-defibrillators (ICDs) in clinical practice, concerns exist regarding ICD lead durability. The performance of specific lead designs and factors determining this in large populations need clarification. METHODS AND RESULTS The Medline, Embase, and Cochrane Collaboration databases were searched for studies including ≥2 of the most...
